How can therapists make therapy children friendly and improve results?

Psychotherapy with children is similar to work with adults, but there are some very important differences, differences that are important to how we interpret the treatment research literature. Unlike adults, children rarely see themselves as needing therapy, thus, most treatment referrals tend to be made by parents, teachers, or other adults. It is these adults that identify the goals they would like the therapist to pursue. In a sense, when children are in therapy, the child is often the “patient,” whereas the parent is “the client.” Given the developmental limitations of children, child therapists heavily rely on the adults for the information about the child, which may be inaccurate, influenced by their own feelings, or biased. As well, the levels of agreement among different adult informants reporting on the same child tend to be low (Achenbach, McConaughy, & Howell, 1987). Therapy with children is much different than therapy with adults, so we cannot simply assume that if therapy is effective with adults, it will also be effective with children.

Children under thirteen years of age have little voice in the services they receive. Children are usually placed in therapy because someone else thinks they need it, which leaves them with little control over the process, and not much reason to want to be involved. Because of this, a shocking 40-60% of youth drop out of therapy (Kasdin, 2004). To make up for the lack of self-report measures for children under the age of thirteen, address the issue of dropouts, and to give these children a voice in the therapy they receive, MyOutcomes developed the Child Outcome Rating Scale.

With more children entering therapy all the time, it is important that we listen to what they have to say if we want them to succeed. And instead of asking a child how they are feeling in all aspects of their life by using a numerical system, or even the adult ORS and SRS, they simply slide a bar towards the sad face or the happy face, making it easy for even young children to tell you how they are feeling.
